Waiver of Liabilities

FOOD ALLERGIES AND FLOWERS:

Addy and Ash Bakery is not responsible for any effects from placing fresh cut flowers on cakes. You acknowledge that fresh flowers are not a food product, and may contain pesticides, insects, dirt, or other contaminants. Due to this:

• I will place fresh flowers on the cake at your own risk and responsibility being advised of all the risks and against the codes of health.

• All of cakes, fillings, and frostings may contain or come into contact with soy, wheat, dairy or nuts.

• You acknowledge that you are responsible for informing your guests of this allergy information.

• If non-edible items are placed on the cake, it is the responsibility of the client to remove any non-edible item before serving.

Addy and Ash Bakery is not responsible for any damage or illness associated with non-edible items. Please make arrangements with the caterer or family members to make sure all items are removed before serving. Plan ahead to make sure all items are removed and not served to guests.
I Have read this Waiver of Liability and Assumption of Risk, fully understand its terms and understand that I am giving up rights, including my right to sue.

I acknowledge that I am signing the agreement freely and voluntarily, I am over the age of 18, and intend by my acknowledgement on the order form to be a complete and unconditional release of all liability to the greatest extent allowed by law.
